We are seeking a Chronic Disease Integrated Team Care (ITC) Program Care Coordinator to facilitate the ITC Program in Maningrida and surrounding homelands/outstations, through the Manayingkarirra Primary Health Care Centre. This is a unique position funded by the NTPHN. The ITC Program Care Coordinator will work collaboratively with individuals and families impacted by a complex chronic disease.
The Care Coordinator will develop and maintain close relationships with practitioners in the Maningrida Health Centre and offsite specialists to create individualised care plans that meet the clients’ needs and facilitate access ancillary services as required improving client improving client capacity to engage with the broader health system.
The full-time position will be Darwin based, with a fly-in-fly-out (FIFO) schedule of 2 weeks per month based in Maningrida (Monday to Friday) and the other 2 weeks per month based in Darwin. Other working schedules may need to be negotiated with additional time in community for certain events and if increased client contact is required. Accommodation in Maningrida and flights between Darwin and Maningrida will be subsidised. The contract will be for a period of 12 months with likelihood of renewal for an additional 12 months subject to funding.

- Work within the Chronic Conditions Program in partnership with the multi-disciplinary team to develop high quality care plans
- Provide a culturally safe, competent and comprehensive care
- Develop and maintain a strong referral pathway

Essential Requirements
The role will see you working in a remote Aboriginal Community. All applicants must have an understanding of, and commitment to Aboriginal Community Control, self-determination, and the issues faced by Aboriginal people living in remote settings.

- Current AHPRA Registration and Practicing Certificate as a Registered Nurse
- Current Basic Life Support Certificate
- Medicines Management module (within the last 12 months)
- About Giving Vaccines (or equivalent)
- Min 5 years broad nursing experience post registration
- Experience working in a culturally diverse setting, ideally with experience working in a remote context
- Proven high level of clinical assessment and problem-solving skills, interpersonal communication and conflict resolution
- Knowledge and commitment to the principles of primary health care and understanding of how the social determinants effect Aboriginal and Torres Strait Islander populations
- Demonstrated ability to communicate sensitively and effectively with staff and members of the general public and clients from diverse cultural and linguistic backgrounds
- Manual Driver’s License
- Satisfactory Criminal History Check
- Current NT Working with Children Clearance OCHRE card or the ability to obtain one
